What this site collects, and what it does not.
This policy covers carrollpt.com, the website of Carroll Physical Therapy, 3155 Route 10 East, Suite 112, Denville, NJ 07834. It describes what happens to information you send through this website. It is a website privacy policy. It does not describe how your medical record is handled once you are a patient of the practice, and it is not a Notice of Privacy Practices.
What we collect. One form on this site collects personal information: the appointment request form. It asks for your name, your phone number, your email address, a short note about what brings you in, and the best time of day to reach you. Nothing else is required, and there is no account to create. Our web host also keeps standard server records such as IP address, browser type, and the pages requested, which is how any website works.
How we use it. We use what you send to contact you about an appointment, and to answer the question you asked. That is the whole purpose. We do not sell, rent, or trade your information, and we do not add you to a marketing list because you filled in a booking form.
Who handles it. Form submissions are processed by the company that hosts this website and are passed into the contact management system the practice uses to schedule appointments and follow up on inquiries. Both are service providers acting on our instructions, and they hold your information for us rather than for their own purposes.
Please do not send medical detail through this site. A web form and an email inbox are not the right place for test results, imaging reports, diagnoses, or anything else you would consider protected health information. Tell us that you have knee pain, and then call (973) 366-1600 for the rest. If you send clinical detail anyway, we will treat it with the same care as anything else in your record, but the safer route is the phone.
Cookies. This site does not set advertising or cross-site tracking cookies of its own. Third-party content embedded in pages, described below, may set its own cookies when it loads. You can block or delete cookies in your browser settings, and this site will still work.
Analytics. If we use a website analytics tool, it is to count visits, see which pages people read, and understand where traffic comes from, reported in aggregate. It is not used to identify you by name or to build an advertising profile.
Third-party embeds. Some pages load content from other companies, and when that content loads, your browser contacts them directly and they can see your IP address. On this site those are: Google Maps, for the location maps on our contact pages; an Elfsight Google Reviews widget, which displays our public Google reviews; and Google Fonts, which serves the typefaces. Their own privacy policies govern what they collect.
Retention and deletion. We keep appointment requests only as long as we need them to respond and to keep an accurate record of who contacted us. You can ask us at any time to tell you what we hold, correct it, or delete it. Email info@carrollpt.com, call (973) 366-1600, or write to us at the Denville address above, and we will act on the request. Note that records tied to treatment you have already received are kept as required by law and cannot always be deleted on request.
Children. This website is not directed to children under 13, and we do not knowingly collect information from them through it. If a form was submitted by a child, contact us and we will remove it.
Changes. If this policy changes, the revised version is posted on this page with a new effective date at the top.
